Prior to beginning practice all students are required to complete a Sports Permission Packet. Click on the icon to the right to access the Sports Permission Packet. The packet includes a Risk of Injury Statement, Health History, Athletic Rules, and Emergency Card. Please return these documents to the school nurse when completed. Documents are Adobe Acrobat files for viewing and downloading to your computer.
